About this job
We are looking for a dedicated Endocrinologist or Internal Medicine Physician board-certified in Obesity Medicine to collaborate closely with our health coaches and provide top-notch care to patients. You’ll manage medication adjustments, interpret labs, perform initial patient consultations, and communicate with health coaches to ensure patients receive exceptional care.
Responsibilities
- Provide virtual asynchronous patient care on the Form Health platform, ensuring timely responses (within 24-48 hours M-F).
- Conduct initial consultations virtually to understand patient history and create personalized treatment plans.
- Prescribe and adjust anti-obesity medications as appropriate, and manage other related conditions.
- Interpret lab results and other diagnostic tests to guide treatment.
- Collaborate with Form Health coaches to support patients through their weight loss journey.
- Maintain accurate and detailed patient records within our EHR system.
- Participate in regular team meetings and training sessions to enhance clinical knowledge and skills.
- Adhere to all HIPAA regulations and company policies to ensure patient privacy and data security.
- Minimum patient load of 25-30 patients.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certification in Endocrinology, Diabetes, & Metabolism or Board Certification in Internal Medicine with additional Board Certification in Obesity Medicine.
- Active, unrestricted medical license in multiple states or willingness to obtain additional licenses as needed (multi-state licensure is a significant plus).
- DEA registration.
- Minimum of 2 years of experience in direct patient care, preferably with experience in obesity management or chronic disease.
- Proficiency with Electronic Health Records (EHR) systems and telehealth platforms.
- Excellent communication and interpersonal skills, with the ability to empathize and build rapport with patients.
- Strong organizational skills and attention to detail.
- Ability to work independently and as part of a collaborative team.
- Must reside in the US.
